A few things to note before continuing the next page:

1. PO boxes or virtual addresses cannot be used as business locations.

2. If the business is located at a commercial address in the City, please attach a Zoning Clearance Form.

3. If the business is operating or located at a residential address in the City, please attach a Home Occupation Permit application.

4. If the business is multiple rental addresses, please email [email protected] a list of all rental addresses inside city limits.

5. If you do not know your business's SIC Code, you can look it up here: SIC Code Lookup.
Your business may be regulated by the statewide stormwater industrial general permit if your SIC Code is: 2000-4599, 5015, 5093, or 5171
